Step 1: Open the Fillable form

• Click the form link.
• Select Download (or right-click → Save link as).
• Save the file to your computer (e.g., Desktop or Downloads).

Step 2: Open the PDF

• Open the saved file using:
  • Adobe Acrobat Reader (recommended), or
  • Your browser (Chrome/Edge), or
  • Preview (Mac).

Step 3: Fill Out the Form

• Click into the fillable fields.
• Type your information.
• Check boxes or select dropdowns as needed.

Step 4: Save the Completed PDF

Important: Do not just close the browser tab — this may not save your changes.

• Click File → Save As
• Rename the file (e.g., FormName_Completed.pdf)
• Choose where to save it
• Click Save

Step 5: Upload the Saved File to the online form below

• Fill out your information in the form below 
• Click Upload.
• Select your saved completed PDF.
• Submit.
